Department OverviewThe Relief Medical Social Worker is part of a team that primarily covers various open shifts across the Care Management Social Work team. The hours can be variable, with some occasional full-time coverage needed such as for a leave of absence. Shifts needing coverage can be during the day, evenings or weekends or overnight. The Relief Medical Social Worker, as a member of a multi-disciplinary team, has the primary responsibility for assessment, counseling, care coordination, information/referral, crisis intervention, patient advocacy and associated services to patients, families, significant others and involved parties/agencies.
