Under the direction of the Associate Dean of Students, the Clinical Care Coordinator provides intervention and case management services designed to promote individual student success and retention. This is accomplished through effective case management and coordination of care between campus and community resources. The Clinical Care Coordinator conducts intervention and case management for students experiencing academic, behavioral, medical, mental health, and other personal concerns.

The Clinical Care Coordinator serves as a member of the Student Support Network (behavioral intervention team) and supports an ongoing caseload of students who benefit from a systems approach to student success and retention. Duties include triage, assessment, intervention, crisis management, and support/care plan development. The Clinical Care Coordinator conducts outreach, education, follow-up, and communication with campus stakeholders, including faculty, staff, students, and family members. The Clinical Care Coordinator engages in ongoing needs assessment, intervention, documentation, and tracking.

The Clinical Care Coordinator serves as an advocate for students' many and various support needs. While the Clinical Care Coordinator does not provide clinical treatment or mental health counseling within the context of a psychotherapist-patient privileged or other clinical relationship, they do provide a vital support program and resource designed to meet the personal, developmental, and education needs of SOU students. Clinical Care Coordinators should be capable of recognizing mental health warning signs and similar issues so they can refer students to clinical practitioners or assist them in gaining access to the numerous university and community resources beneficial to students' mental health, medical, social, and academic needs, among others.

Direct Service, Coordination, and Case Management:

Manage a large and evolving caseload of at-risk students to ensure appropriate utilization of campus and community resources.

Provide direct-service clinical support to promote individual success and retention.

Consult with the Director of Students, Associate Dean of Students, and members of Student Support network regarding student crises.

Partner with students and family members in determining the best possible support/care plan.

Communicate with administrators, faculty, staff, advisors, and coaches as required.

Collaborate with Student Health and Wellness to support case management of complex care.

Provide structured behavioral interventions as assigned.

Follow policies, procedures, and protocols when addressing issues of concern.

Understand and utilize informed consent and information releases, including FERPA releases.

Provide excellent documentation and follow up.

Related Other Duties, Training and Outreach:

Facilitate student support groups

Provide individual and group outreach and support for historically underserved populations and veterans, reentry students, students in recovery, and students formerly in foster care.
