Rutgers, The State University of New Jersey is seeking a Habilitation Counselor for the Housing & Urban Development – Veterans Affairs Supportive Housing (HUD-VASH) Program at Rutgers, University Behavioral Health Care.
The primary role of this position is to provide community outreach, case management linkage to services and housing placement services to homeless veterans who are referred by the Department of Veterans Affairs.
Among the key duties of this position are the following:
Supports and empowers veterans to advocate for appropriate resources based on assessed needs.
Understands the impact of culturally unique variables and intervenes appropriately, as evidenced in case conference and supervision.
Performs on-call duties, as assigned.
Identifies housing opportunities for veterans based on veteran preference.
Accompanies veterans to VASH voucher session briefings, medical appointments, and other appointments, as needed while working with veterans to foster independence and a sense of self-determination.
Makes referrals to appropriate community resources as reflected in clinical records and discussed in team meetings.
Maintains clinical and administrative documentation standards as mandated by UBHC policy and other applicable accreditation and certification entities.
Develops and applies knowledge of trends and advancements in the field of practice.
Keeps abreast of all pertinent federal, state and Rutgers regulations, laws and policies as they presently exist and as they change or are modified.
Minimum Education and Experience:
Bachelor's Degree in a mental health field plus one (1) year of related experience.
Applicants possessing a degree in Social Work must have an active New Jersey certification/licensure as required by the State Board of Social Work.
Valid driver's license with a clean record. Must be able to drive throughout the state of New Jersey in order to outreach homeless veterans.
Equivalent education, experience and/or training may be substituted for the degree requirement.
Experience working with veterans, homeless individuals, Public Housing Authorities and housing placement preferred.
May work at least one (1) evening per week and one (1) weekend day per week.
Security/background investigation via NACI (National Agency Check with Written Inquiries) and a SAC (Special Agreement Check) which includes electronic fingerprinting for all contracted staff working with Veterans.
U.S. citizenship or legal resident status will be verified as part of this process. Each staff member must complete a credentialing process at the local VA medical center (i.e. Vet Pro, if they possess a clinical practice license).
Vet Pro is necessary for all licensed, registered or certified clinical staff members (e.g. LCSW, LPC, RN, CADC).
